So you want to change your name? And preferably gender as well. Well here is how!
Deedpoll
A deed poll is a legal declaration that you intend to use a new name in the future. This will allow you to update your ID documents, and record with companies and organisations. Some companies will offer to create a deed poll for you, for a small cost, but you can create a free deed poll using Free UK Deed Poll website or the template on the GOV.UK website.

NHS
The NHS supports updating your name, title, or gender even if you have not “undergone any form of gender reassignment treatment”. Most GP surgeries will be able to change your details if you ask at reception, but some might be unaware of the correct process they need to follow.
The following NHS guidance will explain to them how to proceed: PCSE (gender reassignment).
The process will involve a new NHS record being created for you, with your new name & gender. You will also receive a new NHS number. Your existing health records should be moved into your new record, but you should confirm this with your GP practice if you are unsure.
